I found this image online from the Amp Garage. Would this diagram work? I already removed the Death Cap.

yes, that is a good diagram. It does assume a fender style layout, but conceptually it is the same.

The "deathcap" does not have to be removed. I have been replacing mine with safety rated capacitors.
like this one:

basically they are made so that when they fail they go open, rather than shorting. no more death.

Is there any reason to have the cap on the polarity switch? Does the installation of the 3 prong defeat the purpose of the polarity switch?

Yeah, adding a 3prong plug does make the cap unnecessary... Assuming that the amp is always plugged into an outlet that is grounded or has the ground hooked up and you have a power conditioner. If not, the cap can still be useful.

Here is the 3 prong wiring, wanted to make sure this was correct.

The black wire from the new AC cord goes to the rear fuse holder tab, and to the polarity switch - the side with only one contact.

The AC cord white wire connects to the black wire in the V4 which runs to the other side of the polarity switch and the blue/white wire that goes to the transformer.

Green AC wire grounds to the PT nut.

white wire in the V4 connects to the other end of the fuse holder and goes to the power switch.

The White wire from the AC is heat shrink'd together with the black wire (which was already in the V4) and it runs to the second to right spot on the terminal strip. It hooks up with that blue/white wire.
